The role of the Medical Social Worker is to work with the interdisciplinary team to ensure a safe discharge of all adult patients. Duties include but are not limited to: · Identifies patients with barriers to a safe discharge plan. · Completes psychosocial assessments on all patients. · Documents all clinical assessments, notes, and referrals in Epic. · Completes screens and coordinates all new skilled nursing facility and rehab facility referrals. · Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault. · Educates patient/family about post-acute care options and the role of choice. · Educates patient/family about managed care and payor issues This is a fu...
