Job Summary This is a full-time professional position. This position will assist in providing Palliative Care Consultation to patients in the hospital setting. This position will provide direct patient care on an ongoing basis. This position will work with the Director of Inpatient Palliative Medicine to educate hospital staff and community physicians about palliative care, pain and symptom management, communication around end of life decision making, and other programs available within Transitions LifeCare. The schedule for this position is Monday-Friday 8:30am-5:00pm; weekends and holidays as needed/scheduled. Responsibilities what you do: - Provide Palliative Care Consultation to patients in the hospital. - Provide direct patient care on an ongoing basis as directed by and under the clinical supervision of the Director of Inpatient Palliative Medicine; utilizing pain and symptom management interventions that are based on established hospice and/or palliative care guidelines and standards. - Attend IDG meetings as appropriate - Document in patient medical record to meet federal, state, regulatory, and agency guidelines. - Educate hospital staff and consult with the community phys...
