Assistant Practitioner | 10433 | permanent contract | 37 hours per week | £32,061 to £33,699 per annum | Grade G | King’s Lynn We have an exciting opportunity for an Assistant Practitioner to join the Western Discharge Social Work Team within Home First Hub (West D2A Team). The team work primarily with older people and adults with a physical disability in West Norfolk. The Integrated Discharge Team is an integrated multi-disciplinary team of social care professionals, health professionals, and discharge planners who triage patients and provide them with information on accessing services aiming to support their discharge from hospital. In this role you would be undertaking Care Act assessments / D2A Planning Form to aid discharge that promote independence with a strengths-based approach and consider Carers Assessments and looking at supporting people to manage their needs supported by Personal Budgets. You will work to achieve a safe discharge plan that is considered the ‘best outcome’ for the service user.
