Overview Deliver Compassionate Care. Support Families. Enhance Patient Comfort. We are seeking a dedicated Continuous Care Licensed Practical Nurse (LPN) to join our team. Reporting to the Executive Director/Administrator or Director of Clinical Services, and working under the direction of a Registered Nurse, you will provide continuous nursing care to hospice patients in facility or home settings. Your role is vital in monitoring patient conditions, managing acute symptoms, and supporting families during critical times. Shift/Schedule: 3pm-11pm: Key Responsibilities: Provide skilled bedside nursing care following the patient’s plan of care and under RN supervision. Monitor patients actively, report changes in condition, and manage acute medical symptoms to achieve palliation during crisis periods. Administer medications, monitor vital signs, and evaluate patient and family needs. Assist with plan of care preparation and execution, including infection control and symptom management. Document patient care activities and communicate findings with interdisciplinary teams and physicians.
