Description JOB SUMMARY: Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, works within a designated program’s self-directed work team, provides a broad spectrum of dietetic services to clients of the Solid Organ Transplant or Bone Marrow Transplant Programs. Applies extensive knowledge, skill and clinical experience to assess, plan, implement and evaluate nutrition goals and care plans for clients throughout all stages of the transplant process. In addition, utilizes advanced clinical techniques such as knowledge of transplant treatment modalities and principles of nutrition support. Participates in the development of dietetic practice guidelines and procedures for the hospital and provincial agencies. Initiates practice-based research projects in collaboration with the health care team. Acts as a resource person to other Dietitians and health care professionals regarding questions and issues related to the nutritional management of clients pre or post transplant.
