Job description

Overview We are seeking a Discharge Planner, sometimes referred to as a Hospital Discharge Planner, Patient Care Coordinator or a Patient Services Coordinator in our Social Work Services Department. As a Discharge Planner, you will support patients, families, and the healthcare staff. The Discharge Planner work goes beyond clinical care to address the psychosocial, emotional, and practical needs of patients during hospitalization and after discharge.
