Case Manager – Older Adults & Adults with Disabilities (Full-Time) East Bay Community Action Program (EBCAP) Location: Riverside, RI | Hybrid Eligible (up to 2 remote days/week) About the Role: East Bay Community Action Program (EBCAP) is seeking a compassionate and organized Case Manager to support older adults and adults with disabilities across Rhode Island. This role is ideal for someone who is passionate about person-centered care, enjoys working in the community, and thrives in a collaborative, mission-driven environment. As a Case Manager, you’ll provide unbiased care coordination and case management services to individuals enrolled in Medicaid, Cost Share (CNOM), and Adult Protective Services (APS). You’ll work closely with participants, families, service providers, and internal teams to promote safety, independence, and overall well-being.
