Description Schedule: Monday-Friday 8am-5pm SUMMARY: The TCM will assist people receiving services in sustaining recovery and gaining access to needed medical, social, legal, educational, housing, vocational and other services and supports. The TCM’s primary focus is on individuals with complex or serious mental health, substance use and co-occurring conditions. TCM will support individuals during critical periods, such as acute episodes, care transition and navigating complex healthcare and social services systems. This also extends to supporting people with high risk due to health-related social needs, suicide or overdose and transitioning from various levels of care.
