Job description

Job Purpose & Position Overview This position is responsible for providing case management services to patients utilizing the Crisis Response Program and the Winnebago Behavioral Health Program. Case Manager is responsible for assessing the physical and psychosocial needs for patients in crisis, needing support to stabilize and remain stable mentally and physically. The Case manager will develop and implement interventions to meet identified needs. This position monitors patient compliance with specific medication instructions and appointment guidelines and formulated treatment plans, educates patients and their families on their specific processes, and provides ongoing assessment, monitoring and re-evaluation of health related needs.
