JOB DESCRIPTION: SUMMARY OF POSITION Provide individual, family, and group counseling services to individuals in a community-based setting. Provide written documentation about clients’ progress as well as communicate collaboratively with team and community partners. This role will work with community partners and local stakeholders to provide comprehensive care in the community. As a part of the community based team, the Therapist will meet with individuals in their own environment such as in the client’s home, hospital, the office, schools/work, etc. ESSENTIAL DUTIES & RESPONSIBILITIES: Reports to work as scheduled on a daily basis and performs duties as assigned by supervisor to ensure the smooth operation of the department.
