Clinical Practice Pharmacist - Solid Organ Transplant (Webster Pharmacy) - ( 2602839 ) Description EDUCATION & EXPERIENCE: Minimum Qualifications: - Bachelor of Science in Pharmacy or Doctor of Pharmacy degree. - Completion of an ASHP accredited post-graduate year 1 residency program or 3 years of related experience. Required: - Licensed to practice pharmacy in the state of Texas or eligible for licensure from the Texas State Board of Pharmacy. Must be licensed with the Texas State Board of Pharmacy within 90 days of hire. PREFERRED QUALIFICATIONS: Doctor of Pharmacy degree: PGY1, PGY2 in Solid Organ Transplant or equivalent specialty experience JOB DESCRIPTION: The Clinical Practice Pharmacist provides advanced medication therapy management and direct or consultative patient care services across assigned practice areas.
