Responsibilities: Qualifications: - Main Office location: Westwood, MA - Service Area: Westwood & Boston areas - Job Type: Part-time - Schedule: Saturday and Sunday 8:30 am – 8:00 pm - Initiates appropriate preventive and rehabilitative nursing procedures. - Administers medications and treatments as prescribed by the physician. - Initiates the plan of care and makes necessary revisions as patient status and needs change. - Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM) - Assesses patient’s needs through triaging and providing in-home care when needed. - Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient. - Responds to inquiries and concerns from patients, their caregivers, and their medical team. - Consults patients an...
