In cooperation with the participant or their designated representative, determine the selection and provision of services, establish their frequency and duration, and secure approval for each service from the participant or their representative as well as from physicians in accordance with specified requirements. Ensure the care plan receives final approval from the participant prior to implementation. Meet face to face with the candidate and allies to complete a NFLOC with Freedom of Choice to maintain participant’s eligibility.
