Location: Multiple Locations (Northern, Southern or Western suburbs The Role: This is a part-time, multi-site clinical position tailored for a Nurse Practitioner who excels in geriatric assessment and preventative care. In this role, you will lead Annual Wellness Visits (AWVs) in your region, ensuring our geriatric population receives comprehensive, high-quality screenings and personalized prevention plans. You will: - Conduct comprehensive Annual Wellness Visits at various Assisted Living, Independent Living, and Long-term Care facilities. - Perform detailed cognitive screenings, functional assessments, and fall risk evaluations. - Lead medication reconciliation and facilitate meaningful Goals-of-Care discussions. - Develop and update patient-centered Care Plans based on AWV findings to address chronic conditions and preventative health goals. - Identify and enroll eligible patients into Chronic Care Management (CCM) programs, ensuring they receive the continuous support necessary for complex health needs. - Ensure thorough documentation and clinical coding to capture all wellness measures What Makes This Role Unique: Specialized Clinical Focus AWVs provide patients with a no-cost...
