Job description

We are seeking a community-based Blended Case Manager to join our team in Colmar, PA serving clients in Montgomery County. The Blended Case Manage r is responsible for developing, coordinating, and implementing services for children and adult consumers as identified in the goal-oriented service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life.
