City/State Virginia Beach, VA Work Shift: Rotating Overview: Sentara is hiring a full-time Pharmacy Technician II Transition Specialist for our Outpatient Pharmacy "Meds to Bed" Program at Sentara Virginia Beach General in Virginia Beach, VA. Location: Sentara Virginia Beach General Hospital- Virginia Beach, VA Status: Full-time 40 hours per week Shift: Monday-Friday 8 hr Shifts: Hours: Rotating between 8:45 am and 7:15 pm Sentara To Home pharmacies are designed to help patients transition to home by assisting with medication needs before leaving the hospital. The program is designed to make the transition to home as simple and convenient as possible for patients and their families. At Sentara To Home, medications, home medical equipment, and other healthcare needs can be provided to be prepared after discharge. The Pharmacy Tech II : transition specialist that consults with patients about what they will need to have a successful recovery once leaving the hospital. Prior to the day of your discharge, the transition specialist will visit the patient's hospital room to discuss medication needs and self-care requirements.
