Plays a key role in obtaining information about patients with medical and social needs and ensuring information is sent to discharge vendors. Acts as a resource to assist patients in navigating the health care system. Serves to facilitate linkage to community resources that will enable the patient to obtain optimal health outcomes and transition safely to the next level of care.
