Overview Children and Family Services Care Manager Helio Health's In Community Program (formerly Circare) supports individuals and their families who encounter barriers to their health and wellness, both physical and behavioral, to develop the resources to live a satisfying and naturally interdependent life. Seeking a Care Manager to work in collaboration with a multi-disciplinary team to perform functions that will assist in the care and treatment of youth and families. Qualified candidates will possess skills in assessment, service planning, and care coordination. Care Managers will meet with families in their natural setting – home, school, or community – to develop and coordinate the implementation of a plan of care that identifies the youth’s health and wellness goals in relation to mental health care, physical health care, family education, leisure, and environment. Pay Rate $22.11/hour: In addition to Helio Health’s comprehensive benefits package, this position is eligible for a $1,500 Sign-On Incentive!
