Integrated Care Coordinator (2 posts) | 10355 | permanent contracts | full-time 37 or part-time 30 hours per week | £29,064 to £31,022 per annum pro rata | Grade F | Norwich We have an exciting opportunity for two Integrated Care Co-ordinators to join the team in Adult Social Services, primarily based in Norwich Place. The Integrated Care Coordinators are an innovative role working across Health and Social Care teams, supporting colleagues from across the system to proactively identify people suitable for preventative intervention and support; networking and connecting professionals such as GP’s, Social Care, Community Health, Housing colleagues and more and acting as a resource for information on community-based resources for colleagues across multiple organisations. You will collaborate with many different professionals and services such as GP’s, Social Prescribers, Social Workers, Nurses, voluntary services and housing colleagues. Fostering and promoting multi-agency working; networking and maintaining relationships are key parts of the role. A positive can-do attitude is essential, alongside professional curiosity.
