City/State Chesapeake, VA Work Shift: Multiple shifts available Overview: Sentara is hiring a Clinical Pharmacist at our Proprium Specialty Pharmacy in Chesapeake, VA Location: Proprium Pharmacy, Independence Pkwy, Chesapeake, VA 23320 Status: Full Time (40 hrs/wk): Shift: First: Hours: Rotates 8:00am-5:00pm // 8:30am-5:30pm: The Specialty Clinical Pharmacist serves as a key member of the Ambulatory Clinical Support team, providing telephonic comprehensive medication management services to patients within Sentara’s Specialty Pharmacies. This role is responsible for ensuring safe, effective, and evidence-based use of specialty medications through patient counseling, therapy monitoring, and coordination with providers. By supporting adherence, optimizing therapeutic outcomes, and documenting interventions, the Clinical Pharmacist I helps advance quality care and patient-centered outcomes across complex disease states. *This Pharmacist supports the call center team for Sentara's Specialty Pharmacies.
