Job description

Reports To : Director of Inpatient Pharmacy FLSA Classification: Full-Time Salary Range $135,000 - $150,000 - Responsible for the safe, efficacious, and cost-effective use of drugs through the application of specialized knowledge, skills, and professional judgment. - Reviews and assesses medication orders from authorized providers to identify, prevent, and resolve any potential and actual drug related issues. - Applies expert knowledge of drugs and related protocols to verify physician orders for medications. - Makes appropriate evidence-based, patient-centered medication recommendations. - Contacts nurses and physicians for collaboration and communicates effectively when necessary to verify orders and obtain additional information regarding the order to facilitate care. - Provides recommendations on appropriate therapies or alternatives when appropriate including, but not limited to pharmacokinetic dosing, renal/hepatic dosing, and antimicrobial stewardship. - Assists in monitoring all aspects of clinical/pharmacy services and medication therapies by reviewing patients’ profiles/charts to ensure safe, efficacious, and cost-effective drug therapy for all patients. - Responds to eme...
