Description ATTENTION! $3,000 SIGN-ON BONUS!: Gateway Rehab Center (GRC) has an outstanding opportunity for a Therapist who is responsible for providing substance use treatment and case management services for assigned patients in our Co-Occurring, Center Township, PA location. This position receives regular supervision and instruction. Responsibilities: - Utilizes and promotes trauma-informed treatment through the provision of evidence-based practices. - Participates in developing the patient’s treatment plan and outcomes in cooperation with the treatment team. - Provides direct therapeutic services consisting of individual, group, and/or family therapy dependent on the patient's needs. - Makes ongoing assessments regarding the patient’s progress in treatment and revises the treatment plan according to patient needs. - Completes clinical documentation accurately and in a timely manner. - Establishes and maintains contact and relationships with referral sources related to patient caseload. - Plans for continuing care of patient and makes appropriate referrals. - Monitors and completes insurance authorizations for patient caseload. - Provides referrals for additional care or service...
