Position Summary: Under the supervision of the Manager, the Speech Therapist performs therapy evaluations and delivers treatment services to patients in their homes. Care is provided in accordance with professional practice standards, regulatory requirements, and organizational policies. Essential Responsibilities: Conduct comprehensive speech therapy evaluations in the home setting Develop individualized treatment plans based on patient needs and goals Provide evidence-based therapeutic interventions: Evaluate patient progress and modify treatment plans as appropriate Complete accurate point-of-care documentation: Maintain working knowledge of regulations, payers, and home health requirements Participate as an effective member of the interdisciplin...
