Position Summary : The Community Outreach Coordinator is responsible for overseeing client referrals, scheduling and performing assessments and marketing to referral sources. This position is also responsible for overseeing VA and Private Duty client referrals. In addition, the Outreach Coordinator assists Veterans and families on navigating the VA system and Long Term Care Insurance. This position will also perform internal/external marketing and complete weekly statistical reports pertaining to intake and discharges.
