This position will act as lead physician for a new "Intensive Case Management Program" and will include a separate clinic, collaborating closely with the other free clinics currently at GRACE. We anticipate operating 4 hour in-person clinic one afternoon per week. The lead physician will have administrative and nursing support in the clinic, and oversee continuing care provided by a full time nurse or paramedic working in the field to follow up, monitor and assist patients with their care plan.
