Job Summary The RN - Nurse Navigator acts as the primary point of contact and is responsible for monitoring and coordinating care for cancer patients and families. Individualized assistance offered to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care from pre-diagnosis through all phases of the cancer Core Job Responsibilities: - Act a single point of contact for patients and their families. Facilitates appointments for consults and support services within establish service standards. - Provide information throughout the continuum of care including educating the patient, their family, and caregivers regarding their diagnosis and treatment options. - Assists patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical trials. - Coordinates care logistics for patients as well as communication among providers. - Assist in making the care seamless, continuous and comprehensive - Assess clinical, emotional, spiritual, psychosocial, financial and other patient needs - Assist patients to overcome ba...
