CLASSIFICATION TITLE: Lead Case Manager (Part-time) SALARY: $28.21 - $36.49 DEPARTMENT: Office Of Senior Services Opening Date: 05/07/2026 Closing Date: 05/21/2026 12:00 a.m. FLSA STATUS: Non-exempt - overtime pursuant to Human Resources Policy EMPLOYMENT STATUS: Just cause subject to probationary period GENERAL SUMMARY: Under direction, provides case management services and support to clients. Completes assessments, monitors, coordinates, and links individuals to resources. Determines eligibility, makes referrals, and assists participants in accessing benefits and services. Works with participants to identify and reach goals. Enters client information into client tracking databases. Develops a plan of service based on assessment. The Lead Case Manager will be responsible for providing comprehensive support to older adults experiencing or at risk of abuse, neglect or exploitation. This role will ensure the safety, well-being, and independence of clients by coordinating services, advocating for their needs, and collaborating with community partners to support their overall well-being.
