Department NWV HOME HEALTH - 517151: Worker Sub Type Regular Work Shift First Shift (United States of America) Pay Grade Job Description: This position covers Hampshire, Hardy & Mineral counties. The Home Health Licensed Practical Nurse provides direct patient care in the home setting as a key member of an interdisciplinary team that includes Registered Nurses, Physical Therapists, Occupational Therapists, Speech‑Language Pathologists, Social Workers, and Home Health Technicians. The Licensed Practical Nurse conducts patient assessments and implements the individualized plan of care established by the Registered Nurse to ensure that each patient’s clinical needs are met. This role delivers one‑on‑one patient care and communicates any changes in patient status to the RN Case Manager in a timely and accurate manner. The home health environment is technology‑driven; therefore, the Licensed Practical Nurse uses email as a primary communication tool and completes all documentation electronically using a laptop‑based medical record system.
