Overview Job Purpose: Provide medical nutrition therapy and diabetes education to improve outcomes and quality of life. Essential Duties and Responsibilities: - Provides individual medical nutrition therapy and individual and group diabetes education to meet patient goals, through in person and/or telehealth sessions - Assesses, evaluates, interprets, monitors, and documents nutritional status and needs of patients; uses established standards of care and evidence-based practice guidelines - Counsels and provides tailored nutrition education to patients and their families - Uses age-specific, disease-specific and culturally competent educational materials and counseling techniques to meet patient needs - Documents in electronic medical record to monitor patient progress - Communicates and collaborates with patient’s primary care provider, as well as other specialty services to coordinate care - Attend all mandatory trainings and department meetings, readily bringing own ideas and thoughts into the discussion - Develops, reviews, updates, and implements educational materials to meet the needs of patients and providers - Provide in-service training or presentations for other company p...
