PCHC’s Care Management team is seeking a Social Worker/Care Manager (MHRT/C or eligible) to join our mission-driven, interdisciplinary care teams. In this role, you’ll build trusted relationships with adult patients, coordinate across providers, and connect individuals to essential community resources that support stability and well-being. This role is best suited for someone who is patient, flexible, and energized by navigating complex systems and barriers to care. If you have experience in social work, care coordination, or partnering with community agencies—and value teamwork in a supportive healthcare environment—we’d love to hear from you. This position may work in a Designated Healthcare Facility which is subject to the State of Maine’s immunization requirements for healthcare workers. If hired, you will be required to show a Certificate of Immunization, or Proof of Immunity, against: 1.) Rubeola (measles); Mumps; Rubella (German measles); Varicella; Hepatitis B; and Influenza.
