Description First Choice Hospice is looking for a full time Hospice On-Call Licensed Practical Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the Boise, ID metro area. Choose the schedule that fits your life! Full time - 7 on / 7 off nights or Monday - Thursday 4:30 PM to 8:30 AM The primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.
