Job Description: In partnership with the Director of Assisted Living and Memory Care Director, Med Aides are responsible for assisting with resident medication delivery and management, providing activities of daily living, and observing, documenting, and reporting resident conditions. Essential Duties & Responsibilities: - Preparing and assisting with medications as prescribed by Healthcare Providers to residents in Assisted Living and Memory Care. - Documenting medications given and/or not given in eMAR. - Communicate to the Resident, Healthcare Providers, Family/POA, and Director/Nurse regarding change in medication orders, refill challenges, or missed scheduled doses of medication. - Observing, reporting, and documenting changes in resident conditions in Yardi EHR progress notes and Nurse to Med Aide Communication Log. - Doc...
