Under the general supervision of the Program Manager, the Care Management (CM) Supervisor provides day-to-day supervision of Lakeland Care’s (LCI) care management teams. CM Supervisors are responsible to ensure quality and appropriate care to the members of LCI in accordance with Federal and State requirements and LCI policy. The Care Management Supervisor plays an integral role in promoting critical thinking and supporting the professional development of each care management staff. Responsibilities & Competencies: - Provide consultation and guidance to care management teams on LCI policies, procedures, guidelines and best practices to ensure effective and cost effective member-centered planning. - Engage in a collaborative, interdepartmental approach in risk management to ensure member health, safety and independence while upholding member rights. - Promote positive relationships between care management staff, members, legal representatives, providers, community agencies and internal departments. - Review internal reports and follow up with care management teams to ensure optimum quality care management p...
