Department: 37000 Advocate Condell Medical Center - Cardiac Rehabilitation Status: Part time Benefits Eligible: No Hou rs Per Week: 0 Schedule Details/Additional Information: Days, Monday - Friday 8- and 12-hour shifts- as needed only. This is not a benefit eligible position. Pay Range: $25.30 - $37.95 This position is as needed/per diem at Advocate Condell Medical. Major Responsibilities: Conducting Exercise Sessions: 1)Educate and motivate patients towards positive lifestyle habits including weight management, effective exercise and smoking cessation. 2)Lead patients in appropriate warm up and cool down exercise activity. 3)Monitor the "Target Heart Rate" utilizing ECG telemetry equipment. 4)Update the exercise prescription accordingly based on individual conditioning responses. 5)Coach patients on appropriate and individualized methods of aerobic activity. 6)Prepares and presents educational classes to the exercise participants as well as to the community. 7)Update patient's progress in ITP's, progress report and discharge summary.
