Description Title: CORE Clinician - Per Diem Reports to: Assistant Director of Social Programs Job Objective: Carry a caseload of CORE CPST clients, providing comprehensive support and guidance to individuals facing behavioral, emotional, and/or educational challenges. Conduct intakes and clinical assessments for MHA programs, ensuring that individuals receive appropriate and personalized care. Responsibilities: · CORE CPST Caseload Management: · Provide individualized support and intervention services to clients enrolled in the CORE CPST program. · Conduct comprehensive assessments to identify clients' strengths, needs, and goals. · Develop and implement treatment plans in collaboration with clients, their families, and other involved professionals. · Monitor progress and adjust treatment plans as necessary to ensure clients' ongoing well-being and success. · Intakes and Clinical Assessments: · Conduct intake interviews and clinical assessments for individuals seeking services. · Evaluate clients' mental health, social, and emotional needs to determine appropriate levels of care and intervention. · Collaborate with interdisciplinary teams to develop comprehensive treatment plans t...
