Summary Using a person-centered approach, the LPN works collaboratively within a team to maintain and provide health care for individuals in Residential Group Homes. You will help ensure that individuals with intellectual, developmental, and physical disabilities receive the highest quality care while actively participating in the development of their personal service plans. Monday - Friday during first shift hours. What You’ll Do: As part of an interdisciplinary team, you will: - Coordinate, follow up, and maintain health care services for assigned clients - Develop, implement, and continuously review individualized healthcare plans - Maintain comprehensive medical records and documentation - Transcribe and communicate physician orders and ensure appropriate follow-up and referrals - Communicate significant changes in a client’s condition, upcoming procedures, medication requests, injuries, or ER visits to directors, staff, and families - Interact with doctors’ offices, hospitals, pharmacies, and other medical providers - Manage all medical appointments, including annual physicals, ensuring timeliness and regulatory compliance - Coordinates with the contracted pharmacy all issues...
