Case Navigator works with clients facing eminent risk homelessness to stabilize housing and remove barriers. Responsibilities will include assessing clients' needs, coordinating supportive services, and assisting them in accessing housing resources. Responsible for working with clients as they navigate the program, providing guidance, advocacy, and support throughout their journey towards housing stability. Case Navigator will assist clients in a short-term case management program to resolve issues to create housing stability by assessing clients using the EMPath matrix.
