Job description

The Community Health Worker (CHW) will serve as a trusted liaison for patients who may require follow up coordinated support beyond the medical care provided. This role focuses on improving access to care, reducing health disparities, and promoting overall well-being through outreach, education, and resource navigation. The CHW will work closely with community partners and human service agencies to connect patients with appropriate services and support. .
