About the Heart Optimization & Evaluation (HOPE) Clinic Join the Heart Optimization and Evaluation (HOPE) Clinic at Corewell Health, a multidisciplinary program caring for patients with advanced heart failure. HOPE RNs play a vital role in managing a complex, high-acuity population through education, clinical triage, and close collaboration with providers and specialty teams. What You’ll Do: - Triage symptoms and manage patient concerns to reduce hospitalizations. - Educate patients and families on heart failure self-management. - Administer IV diuretics and perform point-of-care testing. - Collaborate with physicians, APPs, pharmacists, medical social workers, and device specialists. - Support patients using advanced cardiac devices and AI-enabled monitoring tools.
