Schedule Full Time, Day Shift (80 hours biweekly) Hours: Monday - Friday, 8:00am - 4:30pm General Summary: Supervises and oversees the work activities of Case Management social work staff and works in a team leadership model to develop and maintain departmental operations that are consistent with Hospital and System community needs. May carry a caseload and provides services to patients and families including counseling, discharge planning and referral to community services. This position will also oversee social work processes and assist with implementation and education providing support to the staff across the region. Essential Functions: - Assists in selecting staff, trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions. - Assists in developing budgets. - Identifies needs in program planning and facilitates program development.
