Location: Sylvester: SUMMARY: Provides and coordinates patient-oriented pharmaceutical care services including therapeutic drug monitoring, pharmacokinetics drug dosing, drug regimen review and drug use evaluations in all patient care areas in a variety of practice settings throughout the residency. Provides appropriate training and educational support for pharmacy staff as well as physicians and other professional ancillary staff, patients and their caregivers regarding safe and effective drug therapy throughout the residency. Assists in developing and monitoring of pathways, protocols and mechanisms supporting safe, efficacious and cost-effective use of medications. Assists with distributive functions on a regular basis throughout residency program which includes holiday and weekend assignments.
