Description The Integrated Care Team Social Worker provides timely coordination of quality healthcare services to meet individual’s specific healthcare needs in a cost-effective manner; this involves adaptability to meet community members at health centers, in their home or other community sites. Provides and coordinates care with patients and involved people for an appropriate plan that meets the medical, rehabilitative and financial needs of the individual. Works to provide community members support for social determinants of health barriers. Uses motivational interviewing techniques, advocacy skills and knowledge of mental health and community resources to overcome barriers, offers assistance from community organizations, other team members, and care coordination as appropriate. Applies social work methods to provide emotional and mental health support to individuals.
