The Case Manager provides direct nursing services in the patient's home or place of residence. Key Responsibilities: Conduct initial and ongoing assessments of patients' physical, psychological, social, and spiritual needs to develop and implement individualized care plans. Manage and coordinate all aspects of patient care, including medication management, pain and symptom control, and emotional and spiritual support. Collaborate with physicians, nurses, social workers, chaplains, and other members of the interdisciplinary team to ensure seamless and coordinated care.
