Social Worker Permanent: Grade C- £35,412 - £44,075: Golden Hello payment - £1,000 and £5,000 per annum. Join our dynamic Discharge to Assess Team , a frontline service working collaboratively with the hospital and supporting citizens from Birmingham’s most deprived communities. About the Discharge to Assess Service Our team plays a vital role in: - Supporting patient flow across the hospital system - Identifying community-based solutions - Signposting and advising citizens and carers - Conducting Care Act and Mental Capacity assessments - The team help improve health outcomes and experiences for citizens in Birmingham What We’re Looking For: We’re seeking Qualified Social Workers who: - Hold a CQSW, DipSW, CSS or equivalent Social Work England-recognised qualification - Registered with Social Work England - Have experience working with vulnerable adults (hospital experience is desirable but not essential) - Can work effectively in a fast-paced, multidisciplinary environment - Is confident in making decisions under pressure and managing complex ne...
