Summary: Greatly contributes to the success of integrated primary care ensuring overall quality patient care through prevention and wellness strategies. Supports a clinic by managing and coordinating care for their panel of patients. Monitor attribution, identify gaps in care, coordinate preventative services, and make outreach to patients or health care providers to ensure coordination of care.
