Job Summary: Plans and implements specific treatment programs for individual patients according to principles and practices of physical therapy. Collaborates with an interdisciplinary team in the development of the patient's individualized plan of care. Provides direction and supervision to Physical Therapy Assistants, Physical Therapy students, and Certified Nursing Assistants in the area of patient care. Provides program and staff development support. An advanced level of clinical and interpersonal skills are required. Assessment: time frames required by the Agency. measurement. ex) high risk for rehospitatlization tool, falls assessment tool, nutritional screening, screening for abuse and neglect, Oxygen safety, spiritual assessment… contacts the patient's physician with recommendations to maximize patient outcomes. the overall patient plan of care (such as cognitive deficits, family situations, etc.) in the timeframes established by CMS and Agency policy using only Agency approved terminology and abbreviations.
