Description GENERAL RESPONSIBILITIES: The Physical Therapist, working as part of a multidisciplinary team, provides direct patient care to evaluate and treat injury, disease and other conditions of health. Responsible for planning and administering physical therapy treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients. This position functions with a high degree of clinical autonomy, providing physical therapy services independently within the health center setting while collaborating with medical, behavioral health, and care management team members as needed. The role also includes coordinating care and clinical continuity with part-time Physical Therapy staff who provide coverage during designated sessions SPECIFIC RESPONSIBILITIES: - Provides skilled physical therapy treatment plans in accordance with physician orders and/or applicable referral and direct access requirements, by assessing and interpreting evaluations and test results of patients - Formulates comprehensive treatment program by evaluating patient’s past medical history, disease/condition, impairments, disability and functional/developm...
