RN (Case Manager) Job Description: The Case Manager will assess, plan, implement, and evaluate the needs of patients to ensure they receive appropriate services and support. This role involves collaboration with various stakeholders to facilitate effective discharge planning and improve client outcomes. Responsibilities: Advocate for patients’ needs and rights within the healthcare and social services systems., Conduct comprehensive assessments to determine patient needs, healthcare goals and eligibility for services., Coordinate services and resources, including referrals to healthcare providers, community resources, and support services., Delivers termination of benefits letters to patients as determined by third party payers, assuring patient understanding and awareness of rights and obligations.
