Maternal Infant Health Program Community Health Worker Kent County: Job Summary: The MIHP CHW is a non-licensed public health worker who facilitates access to needed health and social services for Medicaid beneficiaries enrolled in the Maternal Infant Health Program by way of health system navigation and resource coordination. The MIHP CHW may provide health education and/or reinforcement of health education topics consistent with established or recognized health care standards and best practices as delegated by the MIHP Care Manager. These services are designed to be person-centered, with a focus on beneficiary empowerment, fostering self-advocacy skills to promote personalized and effective diagnosis and treatment. Essential Functions: - Participates in community events and activities that promote the awareness of health maintenance and disease prevention. - Conducts home visits to determine client and family needs. Assists with the development and implementation of care plans to meet identified needs and provides program services.
