Schedule: Monday through Friday JOB PURPOSE: The Community Health Supervisor (CHS) collaborates with clinic leadership and the Arizona Alliance for Community Health Centers for Social Determinants of Health to develop work flow, policies and procedures for employees who are working with Social Determinants of Health. The Community Health Supervisor serves as the business development liaison, consultant and patient advocate for El Rio Health by leading and monitoring grant funded projects and research pertaining to Social Determinants of Health (SDOH).
